Inc., and/or Jackson Finance Inc., and/or Forrest
Funding Inc., you authorize these entities to procure
credit information from a nationally recognized credit
reporting agency.

may include negotiable checks that may be endorsed to
get the loan represented by the check or may be
destroyed if the customer does not wish to accept the
loan offer.
NON DISCRIMINATORY POLICIES
The Florida and Federal Laws against discrimination
require that all creditors make credit equally available
to all creditworthy customers, and that credit reporting
agencies maintain separate credit histories on each
individual upon request.
JOINT APPLICANTS
For married applicants, where applicable, no provision
of any marital property agreement, unilateral statement,
or court order applying to marital property will
adversely affect a creditor's interest unless, prior to
the time credit is granted, the creditor is furnished a
copy of the agreement, statement or court order, or had
actual knowledge of the provision.

IMPORTANT
INFORMATION ABOUT PROCEDURES FOR OPENING A NEW ACCOUNT
To help the government fight the funding of terrorism
and money laundering activities, Federal law requires
all financial institutions to obtain, verify, and record
information that identifies each person who opens an
account.
What this means for you: When you open an account, we
will ask for your name, address, date of birth, and
other information that will allow us to identify you. We
may also ask to see your driver's license or other
identifying documents.
